The Patterns of Adapting to Health (PATH) underlie  adult motivation to learn about health, perform activities to support health, and take responsibility for managing it -- all critical to success at disease management. 

The dominant PATH of a person with a chronic condition can be the difference between consistent engagement versus deteriorating connectivity and declining engagement.

PATH Engagement Protocals

The PATH approach to improving enrollee engagement and behavior change is to tailor enrollee engagement consistent with each patient’s abilities and health-related biases.   The PATH Engagement Protocols allow clinicians to see the reasons motivating client disconnect and provides tools to reverse it.
