the Health PatternsThe science of the Health Patterns is based on the Patterns of Adapting to Health (PATH) model, developed by Dr. Frederick Navarro. The PATH model is of the oldest and most broad-based models applied in real world health-related settings. The PATH Model is at the forefront of a new understanding of the relationship between the behavior of individuals and the health of populations.
Discovered in the late 1990′s, the PATH model has been applied by hospital systems and health insurance plans throughout the United States to better understand the populations they serve. However, these organizations did not apply the PATH model to improve individual and population health outcomes. This is now the focus of the Health Patterns. |
Our MissionTo improve individual and population health through increased awareness of the complex forces shaping adult health behavior. Through the use of well-researched and validated assessment tools, our mission is to help health professionals and organizations understand how everyday actions, habits, and preferences around health are determined by complex system forces and systematic patterns that shape health behavior, influence health outcomes, and medical costs.
By making these patterns visible and quantifying their effects on health, health professionals, in particular health coaches, are best suited to apply this knowledge to improve adult health. |
